What “done” looks like (the 5 outputs Support Coordination should deliver)

When Support Coordination is going well, you should be able to clearly see the work being done and how it supports your goals. In this section, “done” means you receive practical help that moves your plan from paperwork to real providers, real supports, and real next steps. If anything is missing, it’s reasonable to ask your Support Coordinator to show how each step links to your NDIS plan and your goals.

Your Support Coordinator’s 5 outputs should generally result in the following:

  • Clear Support Coordination plan: A schedule of what will happen, when, and how it helps you.
  • Participant-focused service mapping: Your needs and goals matched to the right support types, locations, and likely providers.
  • Provider search and shortlisting: Options you can compare, including questions to ask and what “good fit” looks like.
  • Choice and readiness for booking: Help to contact providers, understand availability/waitlists, and confirm supports are ready to start.
  • Ongoing connection and problem-solving: Check-ins, reviews, and escalation when barriers come up (for example, price, staffing, or mismatched supports).

Key takeaway: “Done” means you’re not left with only recommendations—you have actionable next steps, provider options you can compare, and supports that are progressing toward being booked and delivered.

To make sure you’re getting value, ask questions like: “Can you show me the provider shortlist and why they match my needs?” and “What are the next steps if a provider can’t start yet?” You can also use My Care Finders to compare provider options (and service styles) before decisions are made. If your plan uses plan management, your Support Coordinator should also help you understand how payments and invoices work, including via MyMoney NDIS (www.planmanager.net.au), so there are no surprises when supports start.
