A Support Coordination check-in is a regular, practical conversation between you and your Support Coordinator to help keep your NDIS plan working for you. It’s usually about checking what’s going well, what’s changed since your last plan, and what you might need next. Your coordinator can help you confirm services are in place, track progress toward your goals, and make sure you still have the right supports (including who provides them and how often).

It’s also a chance to sort out common issues early, such as missed appointments, gaps in services, waiting lists, unclear supports, communication problems with providers, or uncertainty about how a support will help your goal. If you’re changing providers, your coordinator can help you compare options and prepare questions. A check-in isn’t the same as plan management. Similarly, a check-in isn’t a crisis service—if something urgent happens, you should use the pathways in your plan or supports you already have.
