Start by writing your main goal in plain language (for example: “I want to get to appointments more reliably” or “I want to build confidence to join community activities”). Then note the impact if support doesn’t happen—think about daily routines, safety, independence, social connection, learning, or health.
Next, describe the support type you’re actually asking for. Use everyday examples: personal care, household tasks, transport assistance, skill-building, therapy (if relevant), nursing, behaviour support, or help with assistive technology setup. If you’re not sure, that’s okay—focus on what you need help with most often and what gets hardest during the week. This step makes it easier to match you to the right provider and service booking.

Keep your expectations aligned with your plan (for example, whether a support will be paid from core or capacity-building), so you don’t run into surprises later.
