Recognise and Document the Problem

Knowing when your NDIS supports aren't meeting your needs is the first step toward making things right. Common issues include:

  • Missed or Late Visits: Support workers not arriving on time or at all, disrupting your routine and care.
  • Rushed or Incomplete Care: Tasks being done quickly or skipped entirely, compromising your wellbeing.
  • Unsafe Practices: Anything that puts your health, safety, or dignity at risk.
  • Poor Communication: Not being listened to, or important information not being passed on.
  • "Wrong Support Delivered": Receiving a service that doesn't align with the goals or funded supports in your NDIS plan.

What to Document Immediately: Good records are your best evidence. As soon as an issue occurs, note down:

  • Dates and Times: When did it happen? How long did it last?
  • Who Was Involved: The name of the support worker and the provider organisation.
  • Specific Details: Write down exactly what happened. Stick to the facts.
  • Photos or Notes: Where safe and appropriate, take a photo of an unsafe environment or a missed task.
  • Direct Quotes: Note any concerning things said by staff.

Keep these notes in a simple notebook or on your phone. This documentation will be crucial if you need to escalate the issue.
