Week 1: Gather Your Evidence
The foundation of a strong plan review is evidence that shows your current situation and needs. Start by collecting documents that paint a clear picture of your daily life and the impact of your disability.
What to Collect:
- Impact Statements: Write down how your disability affects your daily routine, self-care, mobility, and social participation. Be specific.
- Incident & Risk Logs: Note any recent incidents, near-misses, or safety concerns related to your current supports or lack thereof.
- Professional Reports: Request letters or reports from your GP, therapists (OT, Physio, Psychologist), or other allied health professionals. Ask them to detail your functional capacity, progress towards goals, and recommendations for future supports.
- Support Logs: Keep a simple record of the supports you currently use, including frequency and any gaps where needs aren't being met.
This evidence pack moves the conversation from "I want more help" to "Here is why I need different or continued support."
