Support Coordination Explained

Support coordination is a Capacity Building support that helps you understand and implement your NDIS plan. A support coordinator:

  • Helps you understand your plan and what it funds
  • Connects you with appropriate service providers
  • Coordinates multiple providers to work together
  • Helps you build your capacity to manage your own supports over time
  • Assists during crises or when things aren't working

There are three levels of support coordination:

  1. Support Connection — light touch, helps you find providers
  2. Coordination of Supports — the most common level, ongoing coordination
  3. Specialist Support Coordination — for complex situations requiring specialist expertise
